

Here are a few recommendations to help you prepare for your visit/procedure at Salem Township Hospital.
Remember to bring:
- All bottles of medications you take, including over-the-counter medication, vitamins, and supplements
Please list reasons for taking, dosage, and how/when they’re taken. - Inhaler
- CPAP machine
- For ortho procedures:
- Cane
- Walker
- Crutches
- Brace
- Please provide a list of any allergies you may have to medications or foods. Additionally, please describe the type of reaction you experience and the severity of your symptoms.
- Please provide the names and phone numbers of your primary care physician as well as any specialists you may be seeing.
- Insurance information- please bring current cards
- Identification card
- Your preferences for medical care in case you are unable to communicate (include Advanced Directive or Power of Attorney documents).
- A list of questions or concerns to be addressed during your hospital visit, stay or after a procedure.
- Please refrain from wearing any jewelry or piercings and leave them at home.
Medical Chaperone
Medical chaperones will be utilized for sensitive exams.
Care Partner
Choosing a trusted family member or friend as your care partner is crucial. Make sure to have their current contact information available.
A care partner is someone who:
- Is an adult (18 years or older) and can stay with you for 24 hours post-op if required.
- Will assist you with your care and make decisions about it.
- Will be responsible for updating other family members.
- Will support your recovery both in the hospital and at home.
- Will ensure that your preferences, values, and goals are considered while planning your care.
